I, , hereby have chosen to allow my child, noted above, to participate in an after-school program in the Jefferson County School District 251. I personally accept responsibility and release the District from any and all liability. I understand that by signing this waiver, I take full responsibility for any injury or accident that may happen while participating in soccer during the 2023-2024 school year. I also understand that this waiver releases the District and every school in the district, et. als, from any and all liability or claims for injury, accident, or sickness, including Covid-19. I also permit my child to ride on district transportation for games and/or similar events.
